 This application is to be completed by all applicants for any position (volunteer or compensated) involving the supervision or custody of minors.  It is being used to help the church provide a safe and secure environment of those children who participate in our programs and use our facilities.

The information contained in this application is correct to the best of my knowledge.  I authorize any references or churches listed in this application to give you any information (including opinions) that they may have regarding my character and fitness for children’s work.  I release all such references from any liability for furnishing such evaluations to you, provided they do so in good faith and without malice.  I waive any right that I may have to inspect references provided on my behalf.
Should my application be accepted, I agree to be bound by the bylaws and policies of Christ Community Evangelical Free Church and to refrain from unscriptural conduct in the performance of my services on behalf of the church.

 Children’s Worker Policy Form

 Christ Community Evangelical Free Church believes child sexual abuse is any sexual activity with a child.  It may be violent or non-violent.  All child sexual abuse is an exploitation of a child’s vulnerability and powerlessness in which the abuser is fully responsible for the actions.  It is a criminal behavior for which the child is not personally, socially and developmentally ready.  It includes behaviors that involves touching and non-touching aspects:

 Touch involves:
  -  Fondling
  -  Oral, genital and anal penetration
  -  Intercourse
  -  Forcible rape
  -  Incest
  -  Assault and murder

 Non-touching involves:
  -  Verbal comments
  -  Pornographic videos, papers, and magazines
   -  Obscene phone calls
  -  Exhibitionism
  -  Allowing children to witness sexual activity

 The above description is strictly prohibited as a worker of Christ Community Church.  If violated, a report will be filed with the proper authorities and the worker will be immediately terminated.
 
Volunteers are never to be alone with children without parental permission.
 
 Workers must report any suspected case of abuse to the program supervisor.

 Strict observation of the “Overnight Rule”:
  -  Two adults with child or youth
  -  Parental Permission for child to attend activity

AGREEMENT

 I have read this document and I understand the conduct that I am to have with the children placed in my care through the ministry of Christ Community Church, and I understand what I am to do if I ever observe or have suspicion of sexual abuse.
